Do I need to forward any ports besides 27500 UDP/TCP (the port I'm running QW server on) to the server machine?

You should be good with just 27500/UDP. If you wan't people to be able to spectate games with QTV then you'll need to forward the port of qtv_streamport/TCP.

We have had some trouble with this. People was not able to connect. We had to open all ports from 27500 to 27600.
